Consultation for information and referrals • Evaluation; identify problems, make recommendations and provide solutions • Short term and long term care planning for safe community living and aging in place • Education to families about resources and create plan to resolve stress • Expert identification of reputable Skilled Nursing care and Assisted Living facilities, Home care Agencies and Adult Homes • Assist with hospital short term sub-acute placements and discharge planning • Coordination of services based on individual and or family decisions • Assist with home live in care arrangement • Home environment assessment for equipment access and safety • Collaborate with physicians to monitor and coordinate treatment plan • Coordination of care between family members, doctors and other professionals and service providers • Financial assessments to determine government insurance eligibility • Education about Special Needs Trusts • Referral to Elder care attorney for legal matters • Education about the criteria for Medicaid eligibility • Complete and file Medicaid application • Counseling about completing Health care proxy

ABOUT JANET

Janet T. Smith, LCSW-R, ACSW, C-ASWCM, received her Masters of Social Work degree from Fordham University School of Social Services in New York City. She is a Licensed Clinical Social Worker and a member of the National Association of Geriatric Care Managers. Ms. Smith is also a Member of The national Association of Social Work and the Association for Certified Social Workers. She is a Field Instructor of social work education, certified by Hunter College on the City University of New York

For the past 27 years Ms. Smith has worked with older adults and the disabled adults in health care-specifically sub-acute care rehabilitation and home care. She began her social work career as a direct care social worker assisting individuals and families to cope with the impact of illness and disability.

Ms. Smith has specialized experience in client advocacy, developing a plan of care, educating clients and families about available resources including government and other entitlements, and facilitating appropriate discharge planning for a successful transition back to community living. on their behalf, providing education about government and private resources to meet their care needs and discharge planning for transiting back to community living.

Ms. Smith has served as a hospital and community liaison, a director of nursing home social work and admissions and director of health care public relations. Ms. Smith’s has an extensive knowledge of sub-acute care rehabilitation and home care. As a result of her broad experience, Ms. Smith is knowledgeable about current health care practices and is able to navigate its modern complexities in order to help clients access needed services.

“It is most important that the needs of the individual are met and they feel empowered with pertinent resources, allowing them to function up to their highest capacity, whether they are living at home or in a facility.” Ms. Smith is a staunch advocate for her clients and is passionate about helping to maintain their dignity and honor their integrity.
